Ms. Howell’s 4th Grade Class
August 29, 2014
Dear Parents,

As we move through the first few weeks of school please make sure to check your child’s agenda and homework assignments. Every day I write the days homework on the board and the students also write it in their agenda. I also post the homework on our class blog: howell4bes.blogspot.com as well as upcoming tests, vocabulary words, and study guides.

As you check your child’s Friday Folder, if there are any grades on the left hand side that are 73 or below, please initial the chart AND go over the assignment with your child. Then send the actual assignment back to school, so that I can keep it.

We have had lots of conversation this week about the importance of listening and following directions.  The 4th grade as a whole has had some issues in classroom as well as in specials classes.  Fourth grade requires lots of responsibility and we expect students to be sure they are responsible for their materials and their learning.  Thank you so much for your help and support! I hope that you have a wonderful Labor Day weekend J

Below are just a few reminders/notes that you will find in the Friday folder:
·       On the left hand side of the Friday Folder you will find the Media Center bookmark for September.  All bookmarks are due Friday, September 26th.  Your child can count the 30 minutes that they read each night towards the appropriate books on the bookmark.   Thank you to all of the students who returned their August bookmark today!
·       There is a flyer regarding PTA reflections. The deadline is September 29th, September 30th, and October 1st

Student Medication Reminders

  • All medications must be in the original child-proof container and not expired.  No medications in baggies or envelopes will be accepted.
  • All medications must be brought in by a responsible adult and may not be brought in by a student on a bus,  Each medication brought to the clinic cannot be dropped off or administered until the parent/guardian completes and signs the GCPS "Administration of Medication Request" form for each medication (Mrs. Durio has these forms in the clinic).
  • All prescription medications MUST be accompanied by a current pharmacy label with the student's name, the medication name, the dosage, and detailed directions.  This pharmacy label is the order from the doctor which must be followed to be in compliance with federal law. I will not accept a prescription medication without the above information.  If the parent does not have a current label, they may go back to the prescribing pharmacy to get an additional label printed.  If the parent needs to keep the packaging that the medication comes in and the label is on that packaging, a copy of the label may be made for clinic records, and the original packaging and label returned to the parent.
  • Any medication that is considered controlled (ADHD meds, narcotic pain meds, and some psych meds) must be counted by the adult bringing them in and the clinic worker to verify the exact number that were brought in every time meds are brought it.  There is an additional form that is completed and signed by the parent and the clinic worker verifying the counts each time.
  • If the student has a chronic condition (such as Asthma, Allergies, Seizures, Cardiac Conditions, etc), and brings in meds for the clinic, the parent will also be given the appropriate health management plan to complete in addition to the other above forms for the medication.
  • Students who have Epi Pens will also have an injection permit to sign as well as the health management plan, the med auth request
